Medical

Please complete the following medical and emergency contact form and return to us one month prior to start of session. Due to Massachusetts state licensing requirements, we are unable to accept your child without receiving these forms. An immunization record must be dated within the previous year.
 
Please note due to the prevalence of peanut allergies we ask that you refrain from sending your child with any peanut products.
 
Any special concerns or accommodation needs must be brought to the attention of Allyson Palmer, program manager, at the time of registration.
